A new Community Connector or social prescribing service is now available in Kilbirnie Medical Practice from Wednesday 24th August 2016. This service offers a way of enabling GP's and Practice Staff to refer and support patients with social, emotional or practical needs with non-medical services and activities available in the community to help improve their health and wellbeing.

The Community Connector will provide health, social care and community activity guidance and support to patients and carers to facilitate access. This includes the provision of information, intelligence and support through multiple communication channels including Carena, the award winning Care & Support website www.carena.org.uk.

A Community connector will be based in this practice every Wednesday. They can signpost patients in the right direction and help find local services on a range of non-medical topics to meet their needs and interests, including:

  • Money and benefit advice, debt management and budgeting
  • Local activities and social groups
  • Self-help and support groups
  • Care services and carers support groups
  • Volunteering opportunities
  • Getting into work, training and education.
